For some reason I like her look in the 2nd pic better. I don't know if she had anything actually done but, it's not overdone and not obvious. I think she looks gorgeous
I have to admit, I'm not a Keira fan. I noticed her enhanced lips in Pirates of the Caribbean (and was annoyed by the everlasting pout!!) , but compared to the before, I can appreciate it.
I'm pretty sure she hasn't had a lip job, her lips were like that from her first movie. and if you're going to compare it, it should be with the same facial expression not a smiley one where we can't tell.
I just find her utterly obnoxious with one of the worst, unrefined English accents I've ever heard! Pretty yet average looking girl who sounds & acts awful.
The reason why her lips look smaller in the before is because she is smiling and that stretches the lips making them appear smaller especially since she has a heavy upper lip. Then inn the after she is pursing her lips so obviously they will look bigger

Notice the upper lip? THATS whats differed. Find it quite funny that it went un noticed by so many of you.
Everyone's top lip is thinned out when they smile & appears fuller when pouting.
